Polycystic kidney disease (PKD) is a systemic disorder that is characterized by progressive renal failure, hypertension, and extrarenal manifestations such as cerebral aneurysm. The clinical characteristics of ischemic strokes in patients with PKD have rarely been reported. We present 3 cases of ischemic stroke in patients with PKD. All patients were men and ischemic stroke in their cases occurred at their age of 23, 38, and 59 years. All these patients had early onset hypertension, and one was diagnosed with PKD after the development of ischemic stroke. Ischemic stroke in the 3 cases were subclassified as large artery disease, small artery disease, and moyamoya disease. One of the 3 patients with severe basilar artery stenosis died due to recurrent cerebral infarcts. PKD needs to be considered as a possible contributor of ischemic stroke in young or middle-aged patients with a history of hypertension or nephropathy.
